Patients could receive standard non-platinum-based or platinum-based chemotherapy for the first or second recurrence of OC, with or without bevacizumab. Patients were excluded if they were platinum-refractory, had previously received more than 2 lines of systemic chemotherapy or had maintenance therapy with poly(ADP-ribose) polymerases inhibitors. The primary endpoint of the study was progression-free survival (PFS). The statistical hypothesis assumed the inclusion of ≥ 350 patients to identify clinically relevant differences (reduction in hazard ratio (HR) of ≥ 30 %) in PFS between study arms, with an expected median PFS in the control group equal to 4 months (with <italic>p</italic> = 0.05 and <italic>p</italic> = 0.2).</p> <p><bold>Results.</bold> A total of 354 patients were included, significant imbalances were noted between arms in the frequency of pathogenic <italic>BRCA1/2</italic> mutations (<italic>p </italic>= 0.039), duration of PFI (<italic>p </italic>&lt; 0.001), and administered non-platinum agents. Median PFS was 7.4 months and 3.2 months in platinum-based and non-platinum-based chemotherapy arms, respectively (HR 0.44; <italic>p </italic>&lt; 0.001), and median overall survival was 23.0 months and 16.5 months (HR 0.73; <italic>p </italic>= 0.011). Multivariate analysis confirmed independent effect of platinum-based chemotherapy on PFS (HR 0.57; <italic>p </italic>&lt; 0.001).</p> <p><bold>Conclusion.</bold> The results of this study suggest superior efficacy of platinum-based chemotherapy in the treatment of early recurrences of OC. Prospective studies are needed to confirm these findings.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Платиносодержащая химиотерапия может сохранять активность при рецидивах рака яичников (РЯ) вне зависимости от сроков их развития.</p> <p><bold>Цель исследования</bold> – уточнение роли препаратов платины в лечении платинорезистентных рецидивов РЯ.</p> <p><bold>Материалы и методы.</bold> Нами проведен крупный ретроспективный анализ результатов лечения пациенток в НМИЦ онкологии им. Н. Н. Блохина. В исследование включали данные пациенток с рецидивами серозного или эндометриоидного high-grade РЯ с бесплатиновым интервалом ≤ 6 мес, получавших лечение в период с 2014 по 2024 г. Пациентки, получавшие стандартную неплатиновую или платиносодержащую химиотерапию по поводу 1-го или 2-го рецидива РЯ в сочетании с бевацизумабом или без него, были разделены на соответствующие группы. Исключали пациенток с платинорефрактерностью, ранее получавших более 2 линий системной химиотерапии, и тех, кто прошел терапию ингибиторами поли(АДФ-рибоза)-полимераз. Первичная конечная точка исследования – выживаемость без прогрессирования (ВБП). Статистическая гипотеза предполагала включение ≥ 350 пациенток для выявления клинически значимых различий (снижение относительного риска (ОР) на ≥ 30 %) в ВБП между группами исследования при ожидаемой медиане ВБП в контрольной группе, равной 4 мес (при <italic>p</italic> = 0,05 и <italic>p</italic> = 0,2).</p> <p><bold>Результаты.</bold> В анализ включены данные 354 пациенток, между группами отмечен дисбаланс по частоте патогенных мутаций <italic>BRCA1/2</italic> (<italic>p</italic> = 0,039), длительности бесплатинового интервала (<italic>p</italic> &lt; 0,001), применяемым неплатиновым агентам. Медиана ВБП составила 7,4 мес в группе платиносодержащей терапии и 3,2 мес в группе неплатиновой терапии (ОР 0,44; <italic>p</italic> &lt; 0,001), медиана общей выживаемости – 23,0 и 16,5 мес соответственно (ОР 0,73; <italic>p</italic> = 0,011). В многофакторном анализе подтверждено независимое влияние применения платиносодержащей химиотерапии на ВБП (ОР 0,57; <italic>p</italic> &lt; 0,001) пациенток.</p> <p><bold>Выводы.</bold> Результаты проведенного исследования позволяют предположить превосходящую эффективность платиносодержащей химиотерапии в лечении ранних рецидивов РЯ.
