Background. The high frequency of frailty and sarcopenia in patients with cancer led to the development of «prehabilitation» concept. multimodal prehabilitation includes physical training, nutritional and psychological support before anticancer treatment.

Aim. we aim to present the results of treatment of a patient with frailty and severe sarcopenia who underwent a multimodal prehabilitation program before surgery for colorectal cancer.

Materials and methods. A 81-year-old woman with colon cancer was diagnosed with frailty and severe sarcopenia. The patient underwent a multimodal prehabilitation for 12 days prior to surgery, which included nutritional and psychological support as well as a supervised physical exercise program based on nordic walking.

Results. The patient gained weight. Also, she increased grip strength and gait speed. The postoperative period went uneventfully and the patient was discharged home 7 days after surgery.

Conclusions. multimodal prehabilitation is a potentially beneficial option in sarcopenic patients with frailty and colon cancer who need surgery.

Введение. Старческая астения и саркопения являются частыми спутниками пациентов, страдающих раком толстой кишки. Одним из путей коррекции этих состояний является интенсивная предоперационная подготовка (преабилитация, или предреабилитация). мультимодальная преабилитация наряду с коррекцией сопутствующих заболеваний подразумевает сочетание лечебной физкультуры, нутритивной поддержки и психологической помощи на этапе подготовки к проведению противоопухолевого лечения.

Цель исследования – представить результаты применения мультимодальной преабилитации у пациентки старческого возраста, страдающей резектабельным раком толстой кишки на фоне старческой астении и тяжелой саркопении.

Материалы и методы. у пациентки 81 года, страдавшей раком восходящей ободочной кишки сT2n0m0, диагностированы старческая астения и тяжелая саркопения. В течение 12 дней пациентка проходила комплексную подготовку к операции, включавшую нутритивную поддержку и персонализированную программу лечебной физической культуры.

Результаты. Отмечено повышение массы тела пациентки и функциональных показателей: увеличение силы захвата руки и скорости ходьбы на 400 м. Ранний и поздний послеоперационные периоды протекали без осложнений, больная выписана в удовлетворительном состоянии на 7-е сутки после лапароскопической правосторонней гемиколэктомии.

Выводы. Наличие старческой астении и / или саркопении требует интенсификации предоперационной подготовки, одним из вариантов которой является мультимодальная преабилитация на основе лечебной физической культуры и нутритивной поддержки.
