This article explores evolution of treatment options in colorectal cancer with synchronous metastatic disease, role of surgical approach on different stages of combined‑modality therapy, including “liver first” strategy. Prospects of perioperative and neoadjuvant polychemotherapy in patients with colorectal cancer with synchronous distant metastases to liver, data on possibility of using radiation therapy in treatment of primary tumor and liver metastases, as well as risks and benefits of this approach and available clinical research data are analyzed.

В статье обсуждается эволюция развития лечебных подходов при первично‑метастатическом колоректальном раке, роль хирургического метода на разных этапах комбинированного и комплексного лечения, в том числе и стратегии «сначала печень». На основании данных крупных метаанализов последних 5 лет анализируются возможности проведения периоперационной и неоадъювантной полихимиотерапии у больных раком ободочной и прямой кишки с синхронными отдаленными метастазами в печени, приведены современные данные о возможности использования лучевой терапии на первичную опухоль при данной патологии, а также риски и преимущества такого подхода, приводятся данные клинических исследований.
